DeKalb County Board of Health
The DeKalb County Board of Health partnered with us to promote their LEAD and REACH programs, aiming to improve health outcomes, prevent chronic diseases, and reduce disparities among underserved communities, including Black, Brown, refugee, and homeless populations.
We conducted desktop research to identify and engage over 20 community organizations through online focus groups, surpassing the original target of 10. These sessions provided insights into health challenges, gaps in initiatives, and actionable feedback for the 2025 Community Health Improvement Plan (CHIP).
The campaign increased awareness of the REACH program, added 13 organizations to the Live Healthy DeKalb Coalition, and delivered tailored educational materials. This collaborative, research-driven approach successfully advanced DeKalb County’s health equity goals.
Results
Through research-driven outreach and stakeholder engagement, the LEAD and REACH programs achieved measurable progress in promoting health equity among underserved communities. Surpassing initial targets, we partnered with more than 20 community organizations—double the original goal—to gather critical input for the 2025 Community Health Improvement Plan (CHIP). The heightened awareness of REACH led to the addition of 13 new organizations to the Live Healthy DeKalb Coalition, expanding resources and collaboration for ongoing health initiatives. This holistic approach not only bolstered program visibility but also reinforced the county’s commitment to reducing health disparities and advancing preventive care, laying the groundwork for sustained community well-being.
